This form was drafted for Supervision class at the beginning of my third classroom observation. This document was created by myself and given to a teacher before an observation which was to be used as a pre-observation form that allowed me to ask questions of the teacher so that when I entered the classroom, I was prepared for what I was to supervise in the learning environment of the classroom.
